Insurance

General Information

This page will provide you with basic information about insurance plan participation, referrals and authorizations, non-covered services and balances that are your responsibility to pay, methods of payment, and additional fees that may be assessed.

We have a customer service team to help with questions or concerns you may have about insurance billing or the amount you owe.   If you have questions please call 313-993-4692.

Insurance Plan Participation

University Women’s Care participates with most insurance and managed care plans. We will file claims on your behalf, however it is our position that payment for services rendered is the responsibility of the patient. Claims submitted on behalf of the patient are less co-payments, co-insurance, deductibles and/or non-covered benefits.   Insurance contracts change periodically, and we recommend that you speak with your insurance plan to see if they include us in their network.

Referrals and Authorizations

Often insurance plans require referrals and/or authorizations in order for services to be covered. A referral is permission from your primary care physician and your health plan to see a particular provider or to have specific procedures done. If your plan requires a referral, your primary care physician must provide the referral prior to services being rendered. If you arrive for services without a referral in place, you may be asked to sign a waiver that holds you financially responsible for the services you receive.

Authorizations are often required for procedures. If an authorization is required, our staff will obtain the authorization from your health plan prior to the service. If you have questions about whether a service will be authorized, please call your health plan.

Non-Covered Services

If your insurance plan does not have a contract with us, or the service/treatment provided is not a covered benefit, full payment is expected at the time of service/treatment. If you are a member of an HMO and you elect treatment without a referral or authorization, payment is expected at the time of service.

Methods of Payment

University Women’s Care accepts Cash, Checks, VISA, Master Card, Discover Card, and American Express.

Additional Fees

To provide optimal care it is important that all patients keep scheduled appointments or notify us in a timely fashion if the are unable to do so.  We require 24 hour notice if you are unable to keep your scheduled appointment.  Failure to notify us of your need to cancel will result in a charge of $25.00.

Patients will be assessed a fee of $25.00 for any returned check, and we will be unable to accept personal checks until the account balance and associated services fees are paid in full.  If this is a repeated offense, we will only accept cash or credit card payments for future service.
